Economics of TV commercials?

In a scripted TV show like Law and Order, what are the economics?
1) How many paid commercials typically air in one hour?
2) How much do advertisers pay per ad per viewer (or per thousand viewers, whatever is the commonly accepted metric)?

What are the numbers for cable ads? Presumably reruns of Law and Order on cable cost less per thousand impressions.

I heard that episodes of Law and Order cost $1.5M per hour to produce. How much is recovered on first run, on cable as reruns, on sets of DVD, etc?

Hopefully you can point me to some authoritative sites which analyze this info.
